Polite at all times.
Explained everything.
Able to get movement back at first appointment.
Follow up exercises given.
Could not ask for more to get shoulder working at nearly 100% movement.

If it is necessary to ask such arbitrary questions (parents names, ages and occupations??? just one example) it would be good if doctor could explain why these questions are being asked does not seem relevant to my diagnosis or treatment.
